I know one woman who routinely wears silver hoops with 16 gauge wires
in her ears. She freely admits that her earlobe holes got stretched
when she was younger and wore heavy earrings but when she teaches
classes she recommends using 20 gauge. Maybe your customers who wear
18 gauge wires have larger holes left from heavy earrings too.

I usually look to industry for these sorts of things. Most
manufactured posts and earwires are 20ga in my experience and I have
used that for years. I might bump up a bit in sterling but not
much–maybe 19ga.

In any case, I usually measure in mm. 20ga = approx…8mm
